Documents Required for a Medical Visa

• The online visa application form is available at https://indianvisaonline.gov.in
• A completed and signed online visa application form, with photographs attached (Kindly note that the applicant must provide their own contact details including mail ID and other information)
• Valid Cameroonian passport with at least six months of validity
• A copy of the passport data page
• Two recent passport-sized photographs (2 x 2 inch)
• Proof of confirmed return flight tickets booked in your name.
• A well-prepared referral letter, duly stamped and signed from a recognized medical facility in Cameroon outlining your condition and need for treatment in India.
• An official invitation letter from the Indian hospital where you'll receive treatment, including details of your medical condition and proposed treatment plan and a statement of the responsibility of the patient and the attendant until their departure from India.
• Proof of sufficient funds or a bank statement of three months to cover your stay and medical expenses in India (if sponsorship is available, attach an affidavit along with their ID copy)
• Copies of medical treatment reports from Cameroon (X-ray, CT scan, MRI. Original certificates should be submitted at the consulate for verification)
• Compatibility report of donor and recipient in case of transplant patient
• Yellow Fever vaccination certificate
